Media Release and Club Rules Agreement

I give permission for Kingaroy and District Tennis Association Incorporated to use any visual image or audio recording of me obtained during any and/or all associated tennis activities conducted by Kingaroyand; District Tennis Association Incorporated for the purpose of media release(s) and /or promotional package(s) i.e. newspaper articles, web page promotion.
I hereby indemnify Kingaroy and District Tennis Association Incorporated and their appointed helpers against all damage and/or loss sustained from this date forward. 

I have read, understand, accept and agree to abide by the Club Rules and Rules of Fixture Play should I take part in the club’s fixture programs (a copy of these rules can be provided on request).
