As a way to cut down your administration time we encourage you to use My Tennis to activate online payments and registrations.  However, if you are accepting paper membership forms you can customise this membership form template to assist with collecting your members personal and membership information.

The membership form is a guide for you to use and customise and also contains example membership terms and conditions.  You can adapt these terms and conditions for your club or make changes if you feel are required.

The form also includes the privacy statement which all Tennis organisation’s/clubs should adopt.  It is important members agree to this privacy statement before they are added to any Tennis participation systems including My Tennis.
